The sizing doesn't appear to be correct - did you see the template I uploaded for 4.25 x 11? the eps file you sent is about 1/3 of that size.

Hello. We had these door hangers printed, but the QR code says "The QR code Campaign has been disabled for some reason" when you scan the code. I am hoping you can fix that because we already had these printed! It should point to https://hargrovepest.com/lawns. Thank you!
