A medical home is a multidisciplinary team providing primary care. It is aimed at the entire population of a neighbourhood.

As it is a question of neighbourhood care, I think that well-known monuments such as the local town hall or the church could be symbolized somewhere in the logo (see attachments).
I also put another logo of a medical house that I like (it shows the pluridisciplinary character).
Nevertheless I remain open to your ideas and suggestions.
